Transcripts are provided free of charge.  For security and privacy reasons, transcripts will only be mailed in hard-copy form.  Only the Trainee may make this request, either using the form above, or in writing.  A request to have a transcript mailed to any address other than the Trainee's address of record will be verified before the transcript will be mailed.  No information will be sent via email or fax.
